Proof of Enrollment for DMV

1.  Student needs to stop by the Counseling Office and fill out a request.

2.  Parents or students can also call and request the Proof of Enrollment.

3.  Allow 24 hours to complete your request.

4.  Students can pick up completed form from the Counseling Office.

*Proof of Enrollment forms are only valid for 30 days.

 
 
Summer Proof of Enrollment Request:  If you need a Proof of Enrollment for the DMV during the summer contact the Main Office at 503-353-5812. Check the CHS Website for summer office hours.
