Student registration card, student profile form, transportation request (if applicable), health information and immunization records, age verification, and a home language survey if a language other than English is spoken at home are some of the forms that you will be given for registration.
 
For your convenience, listed below are a few district forms that you will be asked to complete when registering your child, if applicable. You may complete these forms and bring them with you when registering, or pick them up in the school office along with other registration materials. If your child will be entering kindergarten, or any public school for the first time, you will also need to bring proof of age and immunization records.
