Free sports physicals & immunizations @ Providence Milwaukie

Free sports physicals and immunizations for middle school and high school students, will take place Wednesday, July 25 from 4:30 to 7:30 p.m. at the Providence Milwaukie Healing Place: 10330 SE 32nd Ave. This is a drop-in event, so no need to make an appointment. Cal 503-513-8386 with any questions. A parent or guardian must be present to sign am authorization form for youths under 15. Students should bring their immunization records to the event.
