Flu Vaccine Clinics

Please join us at one of our Flu Vaccine Clinics this Fall! 

Things to Remember:

Vaccines are free for most insurance carriers.  Check this list to be sure yours is covered by our clinic provider. If you carry insurance through NCSD, either the PPO or Kaiser, your vaccine is covered. 

*CHILDREN UNDER 9 YEARS OLD can be vaccinated as long as they have had at least two prior Flu immunizations.Please bring proof of prior boosters. Anyone 15 (13 in WA) and under requires parental consent.

*Bring your insurance card as well as this completed form for each person who is receiving a vaccine.
