Health Coverage Tax Documents

2025 1095-C forms will be available for download through your Employee Access Portal by January 31st.  If you do not have the ability to print in your building, you can also request a paper copy by filling out this form. Once your request is received, the form will be mailed to the address on file within 30 days of the request.

How to download your 1095-C Form

1. Log into your Employee Access account
2. On your home page, under “MY PAY”, click the “Pay & tax information”
3. The 1095-C form should be under Tax Documents (make sure you are in the correct year)
