Additional Life - Employees
Employees can elect "Additional Life" for themselves for Guarantee Issue (no health questions) from a minimum of $10,000 up to a maximum of $150,000 not to exceed five times annual salary. Employee may apply for amounts above $150,000 up to $500,000 not to exceed five times annual salary by completing the Evidence of Insurability (health questions).
Additional Life - Spouse
Minimum of $5,000 up to a maximum Guarantee Issue of $50,000. This coverage may not exceed 100 per cent of the Employee's Additional Life coverage.
Additional Life - Child
Minimum of $1,000 up to a maximum Guarantee Issue of $10,000. This coverage may not exceed 100 percent of the Employee's Additional Life coverage.
Voluntary Life Insurance Enrollment
If you are already enrolled in Voluntary Employee Life, Spouse Life, or Child Life, you may be able to increase your coverage. If you elect to increase coverage by more than $10,000 or by more than $5,000 for your spouse, you will be required to complete an EOI (Evidence of Insurability) form. The form will be provided on Standard's website. Also, if you previously dropped or waived enrollment in Voluntary Life benefits, you will be required to complete the EOI.
PLEASE NOTE: You have 31 days from the date you become eligible for benefits to enroll in Life & Disability coverage. All late applicants are subject to medical underwriting approval.
