Informal Cycle
Beginning of School Year:
- Classified Performance Goals (Completed by employee)
End of School Year:
- Classified End of Year Goal Assessment Form (Completed by employee)
- Classified Informal Evaluation Form (Completed by employee)
Formal Cycle
Beginning of School Year:
- Classified Performance Goals (Completed by employee)
End of School Year:
- Classified End of Year Goal Assessment Form (Completed by employee)
- Classified Formal Evaluation Form (Completed by supervisor)
Probationary Employees
Probationary employees are not required to develop goals during the probationary year. Supervisors will note the end of the probationary period for new employees and inform the probationary employee the date of their six-month evaluation.
The Formal Performance Evaluation form will be completed and shared with the employee prior to the end of the probationary period. Probationary employees who meet the requirements of the Standards and Workplace Expectations will earn permanent status following their six-month probationary period. Probationary employees will remain on a Formal evaluation cycle following their probationary period.

Federal Family Medical Leave Act (FMLA) Informational Poster
Below are the OFLA/FMLA forms needed to request a leave of absence. These forms can be emailed to HRinfo@nclack.k12.or.us, or faxed to Human Resources at (503) 353-7378. Contact Human Resources at (503) 353-6000 with any questions.
Step 1: All employees requesting a Leave of Absence need to complete an OFLA/FMLA Leave Request Form:
Step 2: You must have the Certification of Health Care Provider completed by the treating physician and submit it to Human Resources no later than 30 days prior to the anticipated leave start or 15 days after an emergency/unplanned leave.
