All About Me One-Page Profile

The All About Me One-Page Profile is designed for parents and guardians who wish to share information with anyone who spends time with their child, such as members of the IEP team!

The page includes things like your child's:

• name, age, grade, and photo
• strengths and interests
• vision and goals
• challenges and best ways to provide support
• family and community supports

This document can be a valuable tool for teachers, paraeducators, substitutes, and other school staff. Having this information readily available can help improve staff's ability to work with, encourage, and support your child.
