Medication Assistance Form

• Please use this form if your student requires any prescription or non-prescription medication dispensed at school. This form is also for any medications that the student may self administer at school (i.e., inhalers, etc.).

• This form needs to accompany the medication and should be delivered to the school secretary and/or nurse assigned to the student's school.

School Sports Physical Form

• Middle school and high school students who want to participate in sports must have a current sports physical on file prior to participating.  This form must be completed by the parent and physician.
